Procedure requested: Correction of the appearance of the chest
Local objective examination: Minimal adipose layer at the level of bilateral thoracic subcutaneous tissue. On ultrasound, the mammary glands were not hypertrophied
Indicated procedure: Liposuction and bilateral chest liposculpture
Procedure performed: Liposuction, liposculpture and subcutaneous mastectomy
Discussions: Although it was recommended to the patient that the removal of the mammary glands will most likely not be necessary because they were not enlarged so as to require mastectomy, as minimal hypertrophy of 5 mm bilaterally was confirmed by ultrasonography, the patient specified that he also wants a mastectomy. In certain circumstances (excessive local heat or after exertion) he states that the areolas enlarged and sometimes he felt a pain when palpated. This can be a sign of glandular hypertrophy which in time could lead to a significant growth of the mammary glands.
