Before being admitted, you need to bring the necessary documents, such as your ID and medical tests. Additionally, you should come prepared with a personal hygiene kit, including essentials such as a toothbrush, toothpaste, deodorant, nail clippers, and a razor. If you forget them, don’t worry, as we can provide you with a disposable kit. It’s also recommended to bring your personal electronics, such as a tablet, phone, or laptop, along with a reliable charger. We have chargers available at the clinic for both Android and Apple, but they might not be compatible with your devices, so we remind you to check that you have yours with you.

To complete the hospitalization process, you must bring the required documents, such as your ID card, passport, or birth certificate for minors over 14 years old. Additionally, you need to complete and sign the following documents:

  1. The patient form, which can be downloaded from the clinic’s website and should be completed before your arrival at the clinic.
  2. A specific consent form for the type of procedure, anesthesia, and promotional and informational materials.
  3. A specific consent form for the protection of personal data.

Without these completed and signed documents, you will not be admitted. Patients usually fill them out a day before, when they come for the in-person consultation or when they arrive in Baia Mare for accommodation. They stop by the clinic, receive them in an envelope, and then have enough time to complete them during the evening at their accommodation, saving valuable time on the day of the procedure, when things move quickly in the morning.

As for discharge, it is done according to the doctor’s recommendations, whether it’s day hospitalization or extended stay.
We remind you to make sure you understand the approximate hospitalization period and when the discharge will take place. Upon discharge, you should receive the following documents:

  1. A medical prescription, if the doctor prescribes medication.
  2. A discharge note or medical letter, depending on necessity.
  3. A medical leave certificate, if required.
  4. An expense statement.

We remind you that upon discharge, you should have someone accompany you to assist if needed, and if you come alone, don’t forget to inform us, as we will provide you with instructions on how to plan the initial recovery period.
